I understand I am terminating the electric service with New Power Texas at the service address listed above.

More than 2 business days’ notice is required in the event I need to cancel or change this disconnection request. I may be required to enroll for new electric service if the company is unable to cancel my request.

Scheduled move outs can occur at any time on the date I selected above. I am certain that I do not need electricity at all on the date I selected.

I understand I will remain liable for all charges accrued until the disconnection of service is completed by my local Transmission and Distribution Utility (TDU).

IF applicable, failure to provide a valid forwarding address may result in Early Termination Fee(s) as specified in my Terms of Service and Electricity Facts Label document with New Power Texas.
